The signs
A rumble that appears when you press the clutch and goes away when you lift your foot is the bearing. If the noise is the other way round, with your foot off, then it is the gearbox bearing, a different part and a different job. It is worth listening carefully before you take anything apart.
Mechanical or hydraulic
The classic bearing is pushed by the fork, through a cable or a slave cylinder. The concentric, hydraulic one sits around the shaft and is both bearing and cylinder in one part. They do not stand in for each other: check which one your car has.
Changed as a set
The bearing is the cheapest part in a clutch and the one with the most expensive consequences if you leave it. With a new disc and cover, a new bearing goes on, whatever the old one looks like.
What else to check
The fork, the bearing guide and the clutch slave cylinder get looked at too while the gearbox is out. A worn guide eats a new bearing within a few thousand kilometres.
